Health information

Our request form asks about the kind of concern you want help with and is processed by Jotform. We use that answer only to match you with a provider and share it only with the provider we match you with.

Please do not send claim numbers, diagnoses from your records or other medical details beyond what the form asks. Your provider will collect what they need through their own secure process.

Your choices

You can ask us to see, correct or delete the information you sent us by emailing [email protected]. You can opt out of text messages at any time by replying STOP.
